Regarding the processing of personal data

The State Institution "Minsk Scientific and Practical Center for Surgery of Transplantology and Hematology" pays special attention to the protection of personal data during their processing and respects the observance of the rights of personal data subjects.

1. The agreement on the processing of personal data (hereinafter referred to as the "Agreement") is aimed at protecting the rights and freedoms of individuals whose personal data is processed by the State Institution "Minsk Scientific and Practical Center for Surgery, Transplantology and Hematology" on the website http://www.msthps.by/ . This agreement has been developed as one of the measures to protect personal data provided for in Article 17 of the Law of the Republic of Belarus dated May 7, 2021 No. 99-Z “On the Protection of Personal Data” (hereinafter referred to as the Law).

2. The processing of personal data is carried out taking into account the need to ensure the protection of the rights and freedoms of personal data subjects, including the protection of the right to privacy, personal and family secrets, based on the following principles:

• the processing of personal data is carried out on a lawful and fair basis;
• the processing of personal data is carried out in proportion to the stated purposes of their processing and ensures a fair balance of interests of all interested parties at all stages of such processing;
• processing of personal data is carried out with the consent of the subject of personal data, with the exception of cases provided for by legislative acts;
• The processing of personal data is limited to the achievement of specific, pre-declared legitimate purposes. It is not allowed to process personal data that is incompatible with the originally stated purposes of their processing;
• The content and scope of the processed personal data correspond to the stated purposes of their processing. The personal data being processed are not redundant in relation to the stated purposes of their processing;
• The processing of personal data is transparent. The subject of personal data may be provided with relevant information regarding the processing of his personal data;
• the operator takes measures to ensure the accuracy of the personal data processed by him, updates them if necessary;
• Storage of personal data is carried out in a form that allows to identify the subject of personal data, no longer than required by the stated purposes of processing personal data.

3. Personal data is processed for the following purposes:

• ensuring compliance with the Constitution of the Republic of Belarus, legislative and other regulatory legal acts of the Republic of Belarus;
• protecting the life, health or other vital interests of personal data subjects;
• preparation, conclusion, execution and termination of contracts with counterparties;
• conducting surveys and studies aimed at identifying the satisfaction / dissatisfaction of personal data subjects with the medical services provided or the process of obtaining them;
• obtaining and analyzing information that can help improve the services provided;
• carrying out marketing programs aimed at increasing customer satisfaction, building communication with the customer, timely informing about discounts, promotions, campaigns, presentations, personal special offers, etc.;
• carrying out statistical research;
• notifying the subject of personal data about the medical services provided by the Operator;
• communication with the subject of personal data;
• for other lawful purposes.

4. List of personal data processed by State Institution "Minsk Scientific and Practical Center for Surgery, Transplantology and Hematology"

• basic information, i.e. full name, gender, citizenship, date (day/month/year) and place of birth, address/registration/residence information, postal code, information contained in an identity document;
• contact data and information about activity on the sites, i.e. email address, phone (mobile), contacts in chats and other electronic resources, information from the social network to which the subject of personal data has provided access, including the online identifier (ID) your social media account, contacts or list of "friends", as well as information that the subject of personal data has published, making it publicly available, information about the reviews left on the site, cookies, information about activities on the site, date and time of visiting the site, updating and deleting data, incl. using metric programs;
• other data that may be necessary in certain cases or related to the provision of paid medical services (for example, the address of the place of residence or registration, passport data);
• other data that data subjects wish to share with us.

The list of personal data is determined in accordance with the legislation of the Republic of Belarus, taking into account the purposes of processing personal data specified in clause 3 of the Agreement.

5. The processing of personal data is carried out with the consent of the subject of personal data to the processing of his personal data, unless otherwise provided by the legislation of the Republic of Belarus in the field of personal data. By providing personal data of other individuals (relatives, acquaintances, friends, etc.), the subject of personal data guarantees that he has received the consent of these persons to transfer their personal data for processing in accordance with the purposes of processing personal data.

Information related to personal data is not disclosed or distributed to third parties without the consent of the subject of personal data.

6. List of actions with personal data and methods of their processing: the processing of personal data includes any action or set of actions performed with personal data, including the collection, systematization, storage, modification, use, depersonalization, blocking, distribution, provision, deletion of personal data .

The processing of personal data is carried out using automation tools. The processing of personal data is carried out using methods that ensure the protection of such data, except for the following cases:

• in case of depersonalization of personal data;
• in case of public access to personal data; and subject to the established requirements for ensuring the security of personal data, requirements for storage technologies for such data in accordance with the current legislation of the Republic of Belarus.

When processing my personal data, the Operator takes all necessary organizational and technical measures to protect personal data from unauthorized or accidental access to them, destruction, distortion, blocking, copying, distribution of personal data, as well as other illegal actions.

7. Subjects of personal data have the right to:

• withdrawal of consent to the processing of personal data;
• receiving information regarding the processing of their personal data;
• changing your personal data;
• receiving information about the provision of your personal data to third parties;
• require the termination of the processing of personal data;
• require the deletion of their personal data;
• appeal against actions (inactions) and decisions related to the processing of personal data.

The specified rights of personal data subjects can be exercised by submitting applications to the address: State Institution "Minsk Scientific and Practical Center for Surgery, Transplantology and Hematology", 220045, Minsk, st. Semashko, 8. The application can be sent in writing in accordance with the legislation of the Republic of Belarus.

If, in accordance with the requirements of the Law on Personal Data, all the necessary information is not reflected in the application of the subject of personal data or the subject does not have the right to access the requested information, then a reasoned refusal is sent to him.

The subject of personal data may be denied the provision of information in accordance with paragraph 3 of Article 11 of the Law on Personal Data.
